We ask each household to volunteer to help with at least one

Learning Session. Please circle your choice:

October/December/February/March

Fees: $150 /Family,$60/Individual. Fee includes all materials for learning

sessions, textbooks and at-home resources for families, and adult faith

formation materials. Learning Sessions require pre-registration for meal

planning. Meal registration cards will be available at church prior to sessionse-

mail reservations are also accepted. Suggested donations for

meals~~$5/adult, $3/child.

Please return completed form to Faith Formation Office 429 Upham Street,

Melrose, via hand, mail or by the collection basket at Sunday Mass.
